What is a Patient-Centered Medical Home?

In a Patient-Centered Medical Home the pediatric care team works in partnership with your child and your family to ensure that all of the medical and non-medical needs of our patients are met.

Through this partnership, the pediatric care team can help you access, coordinate, and understand specialty care, educational services, out-of-home care, family support, and other public and private community services that are important for the overall health of your child and your family.

The American Academy of Pediatrics (AAP) developed the medical home model for delivering primary care that is accessible, continuous, comprehensive, family-centered, coordinated, compassionate, and culturally effective to all children and youth, including children and youth with special healthcare needs.

Complete Pediatrics, P.C. has also undertaken the National Committee for Quality Assurance’s (NCQA) Patient Centered Medical Home Recognition program.

In September 2021, we achieved recognition for our hard work in facilitating partnerships between our providers and our patients and families.
