Office Policies


Appointments

Scheduling:

We offer a range of providers and appointment time slots to accommodate your schedule. Please call our office to schedule appointments and our front desk staff will accommodate your request to the best of their ability. We are hoping to offer online booking in the future in order to provide greater flexibility to those families who would like to schedule urgent visits after the office has closed. As a friendly reminder, if you are running late or need to reschedule then please contact our office as soon as possible.

 

After Hours:

While we understand that many parents would like to schedule appointments before the office opens, we kindly request that families contact the on-call Nurse Triage service for emergencies only and hold all non-urgent morning calls until the office opens. Doing so allows the on-call clinician to be available to address urgent matters. Our receptionists and front end staff are best equipped to handle appointment requests, as they have full access to the daily schedule that the on-call clinician does not. We appreciate your consideration regarding this request. 

 

Missed Appointments:

Out of fairness to other patients, please call ahead with at least 24 hours’ notice to cancel appointments. We understand that there will be unforeseen circumstances that may prevent you from notifying us in a timely manner, but we ask that you give us as much notice as possible. There will be a $40 "no show" fee if we do not receive 24 hours notice, barring emergencies. The "no show" fee cannot be billed to your insurance carrier. Additionally, we are require that payment be made prior to scheduling your subsequent appointment. We may not be able to continue to care for families who demonstrate recurrent "no show" behavior.

 

Late Arrivals:

Physical examinations and comprehensive appointments are more involved. As such, if you are 10 minutes late for your appointment, it may result in having to reschedule that visit for another day.

 

Medication Refills REQUEST:

Medication refills can be requested by contacting your pharmacy. The request will be forwarded to our office and handled by one of our providers.

 

FORMS

Turn-Around TimE:

Please allow 7 business days for your forms to be filled out. This includes, but not limited to, the following forms:

  • Health Assessment Records  (school physical)

  • Early Childhood Health Assessment Records (daycare physicals)

  • Sports Forms

  • Camp Forms

  • College Forms

  • Asthma Action Plans

  • School Medication Authorization Forms

** PLEASE NOTE: It is your responsibility to make any copies of the forms you may need. If forms are needed before the allotted time frame, a $10 fee will be added to expediate the request.**

 FORM REQUIREMENTS:

The parent/patient portion of the form must be completed before the provider can begin filling out their part. For medication authorization forms the following must also be filled out:

  • Name of the medication

  • Form taken (liquid, tablet, etc.)

  • Strength of medication

  • Quantity taken per dose

  • Dosing frequency

  • Reason for taking medication
